We are committed to protecting your privacy. The information you provide is protected by Commonwealth laws including the Privacy Act 1988.

What information do we collect?

FootForward is an initiative of the National Diabetes Services Scheme (NDSS).  We collect your information when you undertake the FootForward health promotion quiz to provide ongoing information to you about your foot health. We do not record any clinical information or responses to individual quiz questions. We only record the final results (eg, green, amber or red) to inform future communications about your foot health. We may also provide you with information about the NDSS more generally.

At the end of the quiz, you can opt-in to allowing us to record your final results and to receiving ongoing communications from the FootForward program. You can opt-out of receiving communications at any time.

If you provide an email address, this will become your default communication method for information from us.

How do we use & disclose your information?

We use or disclose your information to provide you with access to the FootForward Program, the FootFoward website and related services and for the purposes of the NDSS.

We only use or disclose your personal information to administer FootForward and the NDSS. Your information may be used or disclosed to the following:

• the Commonwealth, as represented by the Department of Health, who fund the NDSS, for administrative or reporting purposes,
• the Australian Institute of Health and Welfare for statistical analysis and research,
• other third parties for the purposes of administering FootForward and the NDSS, including organisations that deliver services on our behalf.

We are committed to protecting your privacy. The NDSS Privacy Policy contains more information about how you can access and correct your personal information held by us.

FootForward, the website and learning management system are operated by Diabetes Australia Limited (ACN 008 528 461) (“DA”), as the administrator of the NDSS for the Commonwealth of Australia (“Commonwealth”).
